The natural hair growth cycle is comprised of three stages: the anagen, catagen, and telogen phases. The visible growth of the hair shaft out of the follicle takes place during the anagen phase. Two cell aggregates have a crucial function in the initiation of this phase: the dermal papilla cells together with dermal fibroblasts and the bulge stem cells (or ORSc cells), located on the outer root sheath of the follicle.

Owing to the important role the ORSc cells play, the following factors can be an underlying cause of hair loss. The ORSc cells are less productive and as such do not fully support hair growth, or the communication between the papilla and the ORSc cells breaks down, reducing the renewal rate of the papilla cells. These circumstances hinder the re-initiation of the anagen phase following the telogen phase, reducing the overall growth rate and leading to hair loss.
